Use ITI’s official template
The VPAT® is ITI’s template; the completed report is an Accessibility Conformance Report. Use the official edition appropriate to your product, market and requested standard. This page provides preparation guidance and an original evidence worksheet, not an altered or rebranded VPAT form.
Define product and evaluation scope
Identify the product version, deployment, features and documentation that were tested. Include the methods and environments used. A report for an older release or a different configuration may not describe what a buyer will use. Record exclusions and explain their effect on the report.
Prepare criterion evidence
For each applicable criterion gather actual results, affected workflows, remarks and limitations. Explain whether a result comes from automated inspection, keyboard testing, assistive-technology evaluation or another method. The report should let a reader understand the accessible features and remaining barriers, not merely see a confident label.
Worked report preparation
Suppose navigation works with keyboard but one modal loses focus. Capture the version and the modal interaction, document its impact and retain a remediation reference. Use the official template’s relevant conformance terminology and remarks accurately. Do not declare full support based only on the navigation test.
Review and maintenance
Ask the product owner and evaluator to review factual scope and evidence. Update the report after changes that affect accessibility. ITI explains that there is no VPAT certification: avoid a badge or marketing claim that implies one. Follow any additional report instructions in the buyer’s solicitation.
